An exciting opportunity exists for a motivated and collaborative Speech Pathologist to join the Transition Care Program (TCP) at Albury Wodonga Health. Working as part of a multidisciplinary team, you will provide high-quality assessment, intervention and care planning to support people transitioning from hospital to home or other community settings.

About the Role
As a Speech Pathologist within the Transition Care Program, you will:
- Provide evidence-based speech pathology assessment and intervention for clients with communication and swallowing difficulties.
- Work collaboratively within a multidisciplinary team to deliver person-centred care.
- Contribute to comprehensive care planning that supports clients to achieve their rehabilitation and recovery goals.
- Facilitate safe transitions from hospital to community-based care.
- Provide education and support to clients, families, carers and staff.
- Maintain accurate clinical documentation and meet operational performance requirements.
- Participate in quality improvement, professional development and service development activities.
About You
To be successful in this role, you will have:
- Qualification in Speech Pathology and eligibility for membership with Speech Pathology Australia.
- Demonstrated ability to assess, diagnose and manage communication, swallowing and oral motor disorders for the relevant clinical populations of AWH using evidence based practice.
- Demonstrate commitment to, and leadership of quality and safety improvement activities.
- A commitment to own professional development.
- Current driver's licence and ability to travel within the service area as required.
Employment Type: This is a permanent part time (4 hours per week) opportunity.
