The Palliative Care Nurse Practitioner will be a valued member of the Wodonga Community Palliative Care service to provide clinical leadership and mentoring and undertake specialist palliative care clinical consultation to enhance the quality of life for a person with a life limiting illness. As a permanent part time role, the Palliative Care Nurse Practitioner will work as a member of a multidisciplinary team which is inclusive of Clinical Nurse Specialists, Occupational Therapist, Social Worker, Grief and Bereavement worker and Volunteer Coordinator, that encompasses support of holistic needs to alleviate suffering of patients and their families in the community setting. The successful applicant will become an active member of a highly supportive team, displaying a commitment to professional growth and development, quality improvement, as well as developing skills in providing quality palliative care treatment and advice. We are actively seeking candidates who meet the following requirements to apply for this exceptional opportunity.
- Current registration & endorsement with AHPRA as a Nurse Practitioner of Palliative Care
- Demonstrated ability to work collaboratively and provide flexibility within the multidisciplinary community palliative care team, as well with local palliative care consultants and specialists, general practitioners, and other health care providers
- Provision of direct and indirect specialist palliative care for patients and their families in the community setting
- Assist in mentoring and development and delivery of palliative care education and training opportunities
- Identification and involvement in palliative care quality improvement activities and research opportunities
- Demonstrate the ability to engage with & promote reflective practice accross the nursing workforce, and engage in clinical supervision
- Current Drivers Licence
