The Clinical Coordinator is a valued member of the Hospital in the Home (HITH) team working in partnership with the Nurse Unit Manager of the Acute Ward.

  • Providing clinical oversight and care coordination of patients accessing HITH. Leadership and management of the service across the following elements: Clinical Governance framework, Risk management and improving performance, financial stewardship and data collection and reporting that reflects an efficient and effective service.

 

We are actively seeking candidates who meet the following mandatory requirements:

  • AHPRA Registered Nurse including Post graduate qualifications in Health Management or working towards.
  • Demonstrated competence in patient centred care and advanced level problem-solving skills.
  • Able to develop and meet operational objectives.
  • Demonstrate ability to work within a multidisciplinary environment.
  • Strong interpersonal and communication skills
  • Demonstrated ability to foster a positive service orientated culture that encourages sound patient care and stewardship, focusing on innovation and productivity improvements.
  • Proven ability to exercise sound judgement in ethical, clinical and management issues
