The Nurse Practitioner Candidate will be a valued member of the Older Persons - Geriatric Medicine and Nursing team, to undertake a tailored training program to achieve endoresement as Nurse Practitioner - Older Persons.

The mentorship and clinical supervision for this role is provided by an endorsed Nurse Practitioner-Older Persons and Specialist Geriatrcian's within the Acute and Sub-Acute operational streams, and governed by AWH Nurse Practitioner Candidate credentialling and supported Nurse Practitioner network collaborations.

The training schedule, incorporates three clinical rotations between, Aged Services Emergency Team (ASET), Acute Geriatric Evaluation Management (GEM), Older Personal Mental Health (OPMH), Sub Acute Specialist Clinics and the Residential Inreach (RiR) service.

The role will provide professional and clinical leadership to the RIR team, ASET, Geriatric Unit, and Acute Geriatric Unit, to specifically;

• Deliver consultancy and support to staff and healthcare providers regarding client management within Residential Aged Care Facilities (RACFs).
• Strengthen clinical confidence, knowledge, and inter-service networks when managing complex presentations requiring specialised services.
• To develop diagnostic capability and advanced therapeutic intervention skills in the care of the older person. 
• Reduce unnecessary presentations of aged care residents to Emergency Departments by improving timely access to primary care clinicians.
• Develop, coordinate, and facilitate clinical education for AWH and RACF staff regarding care of the Older Person.
