Clinical Nurse Consultant, Aged Services Emergency Team (ASET)

About the Role

Aged Services Emergency Team (ASET)  is based in the Emergency Departments (ED) of AWH. ASET Care Coordinators are responsible for providing expert, appropriate and coordinated screening, comprehensive assessment and care planning of people over 60 years of age, with the aim of preventing otherwise avoidable re-admissions to AWH and re-presentations to EDs, and contributing to the effective flow of patients through EDs. 

ASET works with other AWH and non-AWH services to provide clinical / professional leadership in advocating for the care of older people, the development of appropriate clinical pathways, protocols, policy and models of care, which are informed by evidence-based practice and local clinical research. With the expansion of the Albury Emergency Department in 2024, the ASET service will be extending their operational impact, while maintaining an ongoing service across both Emergency Departments. 

The ASET Care Coordinator role (CNC).  

Candidates meeting the following mandatory criteria are encouraged to apply: 

1. Current Registration with AHPRA (Australian Health Practitioners Agency) or other relevant peak professional body.

2. Has at least 5 years full time equivalent post registration experience with at least 3 years full time equivalent experience within the area of gerontology or related aged care streams.

3. Has approved post-graduate qualifications in gerontology or other post graduate qualification related to aged care.

4. Demonstrates a high level of clinical leadership. 

5. Supports and contributes to quality improvement, clinical risk management and research projects within the area of practice and ward / unit / program. 

6. Communicates openly, effectively and professionally with all staff. 

7. Demonstrates consistent and systematic application of the process in the provision of person centred care.
