Aged Services Emergency Team (ASET) is based in the Emergency Departments (ED) of AWH. ASET Care Coordinators are responsible for providing expert, appropriate and coordinated screening, comprehensive assessment and care planning of people over 60 years of age, with the aim of preventing otherwise avoidable re-admissions to AWH and re-presentations to EDs, and contributing to the effective flow of patients through EDs.
ASET works with other AWH and non-AWH services to provide clinical / professional leadership in advocating for the care of older people, the development of appropriate clinical pathways, protocols, policy and models of care, which are informed by evidence-based practice and local clinical research. With the expansion of the Albury Emergency Department in 2024, the ASET service will be extending their operational impact, while maintaining an ongoing service across both Emergency Departments.
The ASET Care Coordinator role is a multidisciplinary position, meaning both Registered Nurses and Allied Health professionals are eligible to apply.
Candidates meeting the following mandatory criteria are encouraged to apply:
1. Current Registration with AHPRA (Australian Health Practitioners Agency) or other relevant peak professional body.
2. Specific post graduate qualification and 12 months experience working in the clinical area of his/her specified post graduate qualification (Clinical Nurse specialist role ONLY)
3. At least five (5) years relevant clinical experience.
4. Demonstrates a high level of clinical leadership.
5. Supports and contributes to quality improvement, clinical risk management and research projects within the area of practice and ward / unit / program.
6. Communicates openly, effectively and professionally with all staff.
7. Demonstrates consistent and systematic application of the process in the provision of person centred care.
Employment Type: This is a fixed-term full-time or part-time role.
